    The most encouraging UC-specific data comes from that 2024 trial of 41 patients with moderate-to-severe, drug-refractory UC: 73.2% achieved a clinical response and 41.5% reached remission two months after two monthly IV infusions of umbilical cord MSCs, with 70.0% still showing a response and 34.2% still in remission at six months — genuinely promising numbers for a group that had already failed conventional treatment.

    A 2019 systematic review and meta-analysis pooling 15 studies (7 human trials, 216 patients) found MSC therapy outperformed standard mesalamine (5-ASA) treatment and reported a 78.7% overall healing rate across single-arm human trials — though the authors were careful to note the studies were small and methodologically inconsistent (Lightner et al., Stem Cell Research & Therapy, 2019).

    A Cleveland Clinic-led Phase Ib/IIa trial of remestemcel-L, an allogeneic bone marrow-derived MSC product injected directly into the colon during colonoscopy, reported that all 12 initial UC patients reached clinical and endoscopic remission by six weeks, alongside a marked drop in fecal calprotectin (a gut inflammation marker) — while placebo-treated patients got worse on the same measures (Lightner et al., Colorectal Disease, 2022). Trials like NCT04543994 continue to enroll medically refractory UC patients to test MSC infusions further (ClinicalTrials.gov). Across all this research, no serious treatment-related adverse events have been reported, though sample sizes remain small.

    Who Might Be a Candidate

    Right now, MSC therapy for UC exists only inside research studies — not as a standard or approved treatment option. People who may eventually be considered for a clinical trial include:

    • Adults with moderate-to-severe UC that hasn't responded to biologics or conventional immunosuppressants
    • Patients facing colectomy (surgical colon removal) who want to explore additional options first
    • Those willing to enroll in an IRB-approved clinical trial with structured monitoring, rather than pay for treatment at a private clinic
    • Patients in stable enough health to tolerate infusion- or colonoscopy-based procedures
    • Anyone who discusses trial eligibility directly with a gastroenterologist or IBD specialist, ideally one affiliated with an academic research center

    Bottom Line

    The research on mesenchymal stem cells for ulcerative colitis is genuinely encouraging — remission and response rates in early trials exceed what many refractory patients see with standard therapy, and safety signals so far are reassuring.
